Download patient informed consent (for patients that you will include in the study)
It is important that we receive a signed informed consent form of you (the participant) and all 10 patients you will include in this study. Therefore we suggest that you print one form for yourself, and 10 forms for your patients, as soon as you decide to participate. All forms can be scanned and sent to dominique.hansen+ebpracticenet@uhasselt.be after you complete the study.
After the study is completed, you will receive Pro-Q-Kine Accreditation points if you participated.
